All jokes aside, we are incredibly lucky to have compassionate and committed team members who work together, support one another, and make a High Five difference for our residents every single day. 💙💛

It's important to me to thank the others who work alongside me. Without all of us, the residents would not get the loving care that they need and deserve. I'm grateful that the staff is supported and appreciated throughout our home. It truly is a home.

An exciting opportunity is available to partner with Jaffrey Rehabilitation & Nursing Center and Derry Rehabilitation & Nursing Center as our Clinical Liaison for the New Hampshire market.
This position is open because of growth and development within our organization—and it comes with a unique opportunity to learn alongside the person currently in the role, gaining firsthand knowledge of our centers, referral partners, and admissions process.
While clinical experience is preferred, we’re focused on finding the right team player who brings the knowledge, skills, and abilities to be successful in this role.
Experience with admissions, long-term care, skilled nursing, healthcare referral relationships, or other clinical settings may all bring valuable perspective. We recognize that great candidates don't always come from the exact same career path, and different types of clinical and healthcare experience will be considered.
If you understand the importance of connecting patients with the right level of care, enjoy building relationships, and want an opportunity to grow alongside two New Hampshire centers, we’d love to hear from you.
